Pancreatic and Periampullary Carcinoma (Nonendocrine)

چکیده
Ductal adenocarcinoma is the most common primary malignant disease of the pancreas and periampullary region. It accounts for more than 75% of all nonendocrine tumors arising in this region, which includes the pancreas, the ampulla of Vater, the distal common bile duct, and the perivaterian duodenum. Pancreatic adenocarcinoma is the most common of the periampullary neoplasms. In the United States, the annual incidence of pancreatic cancer is 9 cases per 100,000 population; it ranks eleventh among all cancers and ninth among nondermatologic cancers. It remains the most lethal cancer, with an overall 5-year survival rate of less than 3% and a death-to-incidence ratio of approximately 0.99. Because more than 28,000 persons in the United States are expected to die of pancreatic cancer in 2000, it is the fifth leading cause of cancer deaths in this country. This chapter largely focuses on pancreatic adenocarcinoma. Ampullary, distal common bile duct, and duodenal adenocarcinomas are less common than pancreatic tumors. They account for 15 to 20% of all periampullary malignant diseases. Arising in the same region, they are associated with similar clinical presentations and management algorithms, but these tumors have different survival rates. These entities are discussed briefly in this chapter.

An Unusually Prolonged Survival in Metastatic Periampullary Pancreatic Adenocarcinoma
Periampullary Cancer is the term used for neoplasms arising from the head of pancreas, ampulla of Vater, distal bile duct and periampullary region of duodenum. Of these, the pancreatic adenocarcinoma carries the worst prognosis with majority of mortalities within the first year. The overall 5 year survival is less than 3%. Studies have shown that pancreatic carcinoma has the shortest median sur...
متن کاملHistopathologic Analysis of Pancreaticoduodenectomy Specimen.
INTRODUCTION Whipple's Pancreaticoduodenectomy has increasingly been used as an appropriate resectional procedure for tumors of the periampullary region which are pancreatic, periampullary, ampullary and biliary tumors. Our aim was to study the distribution and histopathologic features of these tumors and to examine local trends of periampullary neoplasms resected with a PD. METHODS A descrip...

BACKGROUND The early determination of premalignant lesions of pancreas can prevent unnecessary excessive surgical procedures and can reduce morbidity and mortality. Pancreatic intraepithelial neoplasia-3 (PanIN-3) is a preinvasive form of adenocarcinoma (carcinoma in situ). PanINs have not taken place in the literature of radiology yet, it should be considered in differential diagnosis of pancr...
متن کاملLaparoscopic peritoneal lavage cytology and immunocytology in pancreatic and periampullary carcinoma.
BACKGROUND Free tumour cells in the peritoneal cavity of patients with pancreatic carcinoma carry a poor prognosis. Reactive or degenerative mesothelial cells can make cytological interpretation with conventional stains difficult. Detection of the tumour-associated antigens carcinoembryonic antigen (CEA) and CA19-9 may improve detection. METHODS At staging laparoscopy, 22 patients with pancre...
متن کاملExpression of p16 protein and cyclin D1 in periampullary carcinomas.
OBJECTIVE The majority of the tumors in periampullary region are pancreatic and ampulla of Vater carcinomas. The aim of this study was to compare histopathological features of ampulla of Vater carcinomas with pancreatic ductal adenocarcinomas and to determine diagnostic and predictive values of p16 protein and cyclin D1 expression. MATERIAL AND METHOD Tissue samples from pancreatic ductal ade...
